Teeth grinding, also known as bruxism, is a common condition that can cause pain, headaches and damage to the teeth and jaw muscles. Many patients have found relief from their symptoms with botulinum toxin treatment (Botox). However, the situation changed in March 2025, when Kela removed reimbursement for botulinum toxin treatment for bruxism. What does this mean for patients, and when might the treatment still be useful?

Bruxism is involuntary activity of the jaw muscles that can occur both during sleep and while awake (Finnish Current Care Guideline: Pain and dysfunction of the masticatory system).
For many people, bruxism occurs mainly during sleep, but some experience symptoms during the day as well.
Daytime bruxism is often more conscious. It may involve clenching the jaw, pressing the teeth together or tightening the jaw muscles during stress or concentration.
Night-time bruxism, on the other hand, usually occurs without the patient being aware of it.
Bruxism may lead to symptoms such as:
for jaw and facial pain
headache
wear or chipping of teeth
overuse of the jaw muscles
Bruxism is believed to be regulated by the central nervous system, which means that substances affecting the nervous system may increase the risk of symptoms. Examples include alcohol, tobacco products, stimulants, recreational drugs such as amphetamine or ecstasy, and certain medications such as SSRI and SNRI antidepressants.
Some neurological diseases, such as Parkinson’s disease, may also increase the prevalence of bruxism. Although bruxism does not always require treatment, the symptoms can significantly affect quality of life for many patients.
Before starting treatment, it is important to determine what type of bruxism is present. This evaluation is usually done at a dentist’s office. For example, a bite splint may be an effective treatment for patients with rhythmic night-time grinding or mixed bruxism that occurs both during the day and at night.
Research has not shown that bite-related factors directly cause bruxism (Current Care Guideline), and there is no reliable evidence that adjusting the bite reduces bruxism. Similarly, scientific evidence supporting botulinum toxin treatment in bite physiology is limited.
However, in my clinical work I have seen many patients benefit from treatment, particularly in reducing jaw pain, muscle tension and stiffness in the facial area.
Because the scientific evidence for effectiveness remains limited, Kela no longer provides reimbursement for botulinum toxin when it is prescribed for bruxism treatment.
In addition, Kela no longer reimburses botulinum toxin preparations prescribed by dentists. One reason for the stricter reimbursement policy is the 140% increase in botulinum toxin medication reimbursements between 2019 and 2024 under the Finnish Health Insurance Act.

Botulinum toxin – an effective and low-impact treatment
Botulinum toxin (commonly known as Botox®) is a medication that relaxes muscles. When injected into the masseter muscles, it can reduce excessive biting pressure and relieve symptoms for several months.
The treatment:
done at the reception in a few minutes
lasts for about 3–6 months
does not require recovery time
does not affect facial expressions when done correctly
What does the removal of Kela reimbursement mean?
In March 2025, Kela removed reimbursement for botulinum toxin treatment for bruxism. This means that patients now pay for the treatment themselves. The cost of treatment typically ranges from 300 to 500 euros, depending on the required dose and consultation.
Even without reimbursement, botulinum toxin treatment may still be:
a cost-effective option if the problem is long-lasting or severe
invaluable help when a bite splint is not enough or does not fit
Who may benefit from botulinum treatment?
Botulinum toxin treatment may be suitable if:
you have strong symptoms (pain, headaches, tooth wear)
other treatments have not been enough
you want a lighter alternative to or alongside splint treatment
The suitability of treatment is always assessed individually. At our clinic, treatment begins with a consultation where we review your symptoms and treatment history.
Treatment planning always starts with an individual medical assessment.

In summary
Bruxism can be a surprisingly burdensome condition, but you do not have to deal with it alone. Botulinum toxin treatment remains a safe and effective option for many patients, even without Kela reimbursement.

Botulinum toxin has been used in medicine for many years to treat muscle tension and jaw muscle disorders. When performed by a doctor and with correct dosing, the treatment is generally well tolerated. Suitability is always assessed individually before treatment.
How much does Botox cost for bruxism in Turku?
At our clinic, the consultation and treatment visit for bruxism costs 249 € (VAT 0%). The botulinum toxin medication is charged separately.
